Attention Grade 5 Parents & Guardians,
Grade 5 students will be coming home this week with a packet of paperwork for our trip in May to Bryant Pond. Please read this packet carefully and return the 4 documents that require your information and signature. The final page (purple one) is a suggested packing list that you can keep. Below is information regarding each of the forms that need to be completed and signed:
DES Permission form (blue) - Please cut off the bottom portion, sign and return. You also need to indicate if you want your child to participate in the bow and arrow or rifle range. You can only select one.
Bryant Pond Assumption of Risk Form (Green) - Need to sign and return.
Bryant Pond Photo Release (Pink Form) - This need to be completed, signed and returned.
Bryant Pond Health History Form (Yellow) - this is a front and back form that needs to be completed, signed and returned.
These are due back to school by Thursday, April 17, 2025.
